1. Identity & Personal Details
पासपोर्ट व आधारकार्ड (Oḷakhapatra)
PAN card
Recent passport-sized photographs
Janma pramaṇapatra kinva vayaca purava
2. Address & Residence Proof
Electricity bill / Rent agreement / Property ownership document
Ration card kinva matadara oḷakhapatra
3. Financial Status (if earning)
Gelya 3 mahinyance pagarapatraka
Gelya 6 mahinyance Bank statement
Gelay 2 varṣhance Ayakara Vivaraṇapatra (ITR)
4. Alternative Financial Proof (if non-earning)
Pratidnyapatrak or declaration of financial support from a Kuṭumba sadasya
Details of any savings, fixed deposits, or property
Family income proof from supporter (e.g., parent's or sibling's ITR, salary proof)
5. Health & Medical
नोंदणीकृत vaidyakiya vyavasayikakaḍana physical and mental health pramaṇapatra
HIV, Hepatitis B, and Tuberculosis test reports
6. Character and Readiness
2–3 letters of recommendation (character references)
Palakatvachi tayari aṇi dattaka gheṇyachi iccha yybaddala sva-ghoṣaṇapatra
Proof of family or community support (especially for non-earning women)
7. Additional
Kuṭumbakaḍuna na harakata pramaṇapatra (NOC), if applicable.
Ghaṭasphoṭaca hukuma kinva mr̥tyu pramaṇapatra (jara tumhi adhi vivahita asala tara)
Gr̥uha abhyasa ahavala ( will be prepared by a samajika karyakarta )
